Letterator

Letterator is a webapp for creating canned letters and forms. You can use it to put up a “fill in the blanks” letter or form on your site, then have users input their details and (depending on usage) save the completed document in a database, as an email or have the user print it for snail mailing / faxing.

Its still in development, but will be based on PHP and Javascript. Letterator will likely be released for free under the Creative Commons license.
